Dial() needs improvement

This absolutely must be done for the 0.8.0 gem release.

It should receive the caller_id from a hash key argument and execute SET CALLERID appropriately.
It should pass through the endpoint as the user specified it (e.g. "SIP/trunk/15556667777")

Note: this will ultimately be integrated into the call routing DSL.
